    Check if collaborating with an outside organization on this event. This organization may be required to obtain a Liability Insurance Certificate for the event. Please refer to the Building Use Policy.

    The Music Director of the church must approve the use of piano/organ. Add a note for the sexton if requesting tables, chairs or other setup.
  • Please be specific about the number for chairs and tables and type of tables needed and how they should be arranged.

  • Church Office must be informed of any changes/cancellations in a timely manner.
